My lighter ended up in a similar state as this one after I used some crappy butane for a while (I didn't have any vector). I ended up having to bleed it completely and refill with high quality butane. My "sharp" flame returned. Completely agree that this flame doesn't look right. Here's mine:

Oops, missed that! Mine was a 14mm joint, so I'm not sure whether they've shrunk or not. I do agree about the phallic nature of the mouthpiece, though. I couldn't use mine without my wife giving me a lot of crap about the image it presented. ;) Great function, though!
