@Aezhenn is definitively the maker. @fangorn is the owner. I have a similar one : It's a modular pipe made with 2 stems and 2 glass tubes and a head. You can use 1 stem, 2 stems (more 20cm length) or the complete pipe.

I'm talking about the insert glass tube. He have 2 sizes. Stem with 8mm od diameter glass tube : stem with 11mm od diameter glass tube (right):
