    If you look carefully you'll see the end has been heated enough to flow (but not enough to lose shape). Glass is fully stuff, it's technically liquid at room temperatures......just very thick.     The cold end of the stem? Does the crack extend through the 'fire polish' on the end? A crack on the hot end is likely to get worse with heating, on the cold end I doubt there will be an issue unless you drop it. If this is the same stem that whistles you might kill two birds with one...

    My guess is 'too perfect' a hole. Have you tried it loaded? How about with a screen? Disrupting the uniform air flow at the offending location should change its' tune........ Otherwise, it's likely to drive you nuts. OF
