I think its the 15 x 19 mm also because when you tighten the piece down onto the o-ring it gets tighter, so any smaller and it would make it so the stem wouldn't slide in.

Its not a custom piece because it had a roughly cut tobacco screen on the bottom to stop anything besides vapor to get in the cooling unit.

There isn't anything in the cooling unit you can't buy off the shelf. its two of these guys a screw and a few washer and nuts, its amazing how effective it is.
