This is would bring the grasshopper to a whole other level. For now, I'm planning on making a silicone adapter, but I'd much rather it fit by default. 14 is much preferred over 18mm if we have the option aswell, since 14/18 lo-pro reducers are much better than the reverse.

Has a 14mm water piece adapter been considered? A low-pro 14-18 reducer is less cumbersome than a 14-18 expander.

I recommend getting the Stubby version of the GonG stem, which makes the unit itself smaller while leaving the bowl size the same. Keeps the unit compact! :)

I use the O-Ring Mod to keep my stems secure. Sometimes the joints can stick a tiny bit, but I had the same thing happen with my non-Stubby stem. I haven't found it to be an issue worth putting effort into, I find the combination works superbly.
