Yes, I've used the Anvil with both the XL stem and the USA + 3TM (dimpled glass rather than balls) I'd say I like the form factor a little more with the XL stem, but the vapor through the 3TM is cooler and less harsh.

I feel way better knowing I'm not the only one struggling with the XL cap. I don't think I've managed to unscrew the bowl inside the cap, but I've definitely pulled the stem off the oven a bunch of times. There's barely enough oven to grab onto, and it requires a decent amount of force.
