Whoa, hold up. This method (suppositories) does not create the delta-9 version of thc?!

That is HUGE. Many patients, like for chemo etc. can't tolerate medibles, me included (but I managed when necessary!), but need the long acting property of oral administration. Something about that liver transformation of thc doesn't agree with me any more.

My understanding from another medical article is that most of the blood supply from the rectum does indeed go to the liver and is subject to first pass metabolism but that there are other veins there (a separate system, so to speak) that goes to the heart and brain/body before hitting the liver.

There is a “watershed” of veins in the rectum. Collectively they are called the rectal or hemorrhoidal plexus. The majority of the venous flow ( after absorbing cannabinoids ) goes by way of the superior rectal vein into the portal vein, while the inferior veins flow into the vena cava. The portal vein collects most all venous return from the pancreas to the rectum with all the nutrients and enzymes for digestion) where the blood goes on to the liver for what is called “first-pass metabolism.” The majority of the THC is converted to 11-OH-THC, a similarly psychoactive molecule. The vena cava on the other hand goes back to the heart and general circulation before it more slowly reaches the liver for metabolism.

I'm not a doc so can't confirm any of this but I still see this as being invaluable for some medical patients. Now, lot of patients who would want this may not be able to do this, but eating a bit of fatty food about 1 hours prior is supposed to exercise and rather expend the "first pass metabolism" so that cannabis ingested 45 minutes later or so gets by this liver function.

So, perhaps for those who can eat, a suppository of just coconut oil with a suppository with cannabis about an hour or so later might be something to try to see if effects can be enhanced.

Just random thoughts.

It would be awesome if it worked. I know in the struggles of my first wife, towards the end, it was the vomiting and nausea that was the worst. She was on Marinol and even the dropping of the capsule (Think Vitamin E but brown coating and not clear.) with a sip of water could, at times, start the cycle of purging.

On more than one occasion, we would try most everything up to and including Zofran suppositories. For some reason, they did not work very well. I wonder if it is for the same reasons here. If I had to take her to ER because the purging lasted too long, an injection of the Zofran usually worked. (Along with fluids intravenously.) It was weird to me that the suppository did not always work when the same drug injected always did work.

I wonder if cannabis is the same. I remember breaking the cycle once by piercing the Marinol capsule and having the oil just run down my wife's throat. That worked, but may have been a coincidence. If I had all the products available today, I probably would have gotten her a tincture that could go under the tongue.

So I think for Nausea, you might want to look into Nasal Spray, I use this article http://jpet.aspetjournals.org/content/244/1/23.short if cannabis works the same way (OIL being the carrier)
Nasal works faster and avoids first pass (less psychoactive.)

As for all the mean rectal comments, it works, it work for hemmoroids, also make a great anal lube, bums are funs! :p

Yup been there done that too also chlorpromazine suppositories. Nothing like having a nurse shove something up your bum and get no relief lol. I will report back if I give them a try

Then get a nurse that can do it correctly. :rofl:

Any thoughts or scientific evidence that a Transdermal Patch would be as effective as a suppository?

The dispensary I use has a wide variety of patches in both THC and CBD.

yes, but that does not take into consideration- the endocannabinoid system and the cb receptors that absorb cannabinoids in that system... tissues- all tissues have cb receptors so it would be beneficial to know where the endocannbinoids are in relation to where absorption occurs to know efficacy...

Tommy Chong treated prostate cancer with suppositories. Chong, 77, said his cancer, which he described as "in stage one," will be treated with chemo and radiation -- and, unsurprisingly, marijuana, in the form of suppositories.

"I don't tell them I smoke, we don't talk about that. That's a no-no," he said of his doctors. "But as far as the treating it with the suppositories and that, they're all for it."

The FDA website indicates that the organization has not approved marijuana as "a safe and effective drug for any indication."
